Patient Care Records are released by our Privacy Officer in accordance to HIPAA. To request a private health information please complete the request form linked here.

Fees for Fire Incident Reports

​

There is a $30.00 fee for obtaining fire incident reports that must be paid prior to records being released. Payment is made as you submit and complete the form below. You will receive the following:

• Basic Incident Report & Investigation Report (if available) 

        *Photos of the incident will be mailed on a USB if available
